Job Description
Support children and young people in Out-of-Home Care through case management, therapeutic planning, and frontline coaching. Drive positive outcomes by embedding trauma-informed, culturally safe practices and ensuring compliance with standards and legislation.

The role operates across two primary functions:
n
n
18 hours per week supporting frontline staff in the homes to implement case and care strategies.
n
20 hours per week completing case work duties, including planning, documentation, and stakeholder engagement.
n

What You'll Be Doing:
n
n
Lead and review case plans to meet therapeutic and cultural goals
n
Coach and support staff to embed strategies in daily care
n
Engage with families, carers, and agencies to align goals
n
Maintain documentation and ensure compliance with standards
n
Promote safe, nurturing, and culturally respectful environments
n
n
What You'll Bring:
n
n
Diploma (or working towards) in community, youth, or social work
n
Proven case management and stakeholder engagement skills
n
Knowledge of trauma-informed and therapeutic care practices
n
Robust documentation and compliance capability
n
Current full Australian driver's licence and probity clearances
n
