Guided by our vision of "People Living Better Lives", we provide evidence-based, client-centred support that helps individuals, families and communities achieve lasting recovery.

We are seeking a passionate and experienced Case Manager to join our RISE Day Program in Wagga Wagga. This rewarding role supports young people aged ***** years who are experiencing issues related to alcohol and other drug use, helping them navigate recovery through case management, group programs, care coordination and family support.

About the Role
n
Working as part of a small multidisciplinary team, you will:
n

Conduct participant screening, assessments and intake processes
n
Develop and review individual treatment plans and coordinate care
n
Facilitate and co-facilitate evidence-based group programs
n
Provide individual support to participants and their families
n
Support participants to connect with community services and resources
n
Build and maintain referral pathways and stakeholder relationships
n
Complete case notes, reporting and data collection requirements
n
Work collaboratively with counsellors, lived experience mentors and external providers
n
Promote a safe, inclusive and non-judgemental environment for all participants
n
n
From time to time, you may also provide support across other Karralika programs within the region.
