Care Connect Group is seeking a dedicated Care Manager to join our Aged Care team on a 12-month maximum-term contract. You will assess and develop personalised care plans for older Australians, coordinating services across the Loddon Mallee region and working predominantly from home with occasional travel to our Abbotsford office.

You will collaborate with families, service coordinators and providers, monitor budgets, maintain records, and ensure services adapt to changing needs.
