Key Responsibilities:
n
n
Develop individual care plans specifying client issues, goals, nature of interventions, and plans for review.
n
Consult with clients, their representatives, site management, and clinical staff to ensure appropriate implementation of each client's plan
n
Provide clinically appropriate individualised interventions to clients as indicated
n
Review client outcomes at specified intervals in order to evaluate whether interventions are meeting the goals
n
Complete menu assessments where required and produce high quality client reports.
n

What you'll need:
n
n
Recognised tertiary degree – Nutrition & Dietetics
n
DAA registration
n
Driving licence/own transport
n
Full working rights in Australia
n
National NDIS Worker Screening Check (with online NDIS Worker Orientation Module "Quality, Safety and You" certificate)
n
Current season Influenza vaccination
n
COVID vaccinations and Booster
n
Experience working in aged care.
n
