The Support at Home, Aged Care role ensures the delivery of protected, high-quality, consumer-directed care in line with the Support at Home Program *****), Aged Care Act reforms, and Aged Care Quality Standards.

Key responsibilities

Service Delivery & Client Care
n
Ensure delivery of person-centred, goal-focused in-home care services
n
Oversee assessments, care planning, and service coordination
n
Support client independence, reablement, and wellbeing
n
Ensure effective referrals through My Aged Care
n
Monitor client outcomes and service quality
n
Leadership & Team Management
n
Ensure appropriate staffing levels and skill mix
n
Promote a positive team culture and professional development
n
Operational Management
n
Oversee service delivery performance including budgets and utilisation
n
Support implementation of Support at Home funding arrangements
n
Monitor service demand and waiting lists
n
Ensure sustainable and efficient service delivery
n
Quality, Compliance & Risk
n
Ensure compliance with Aged Care Quality Standards
n
Ensure services align with Support at Home Program guidelines
n
Prepare teams for audits and accreditation
n
Manage risk, incidents, and regulatory reporting requirements
n
Health & Safety




n
Promote a safe environment for clients and staff
n
Ensure adherence to workplace health and safety policies
n
Ensure infection control standards are maintained
n
About you
n
Relevant tertiary qualification in nursing.
n
Experience managing community aged care or Support at Home services
n
Strong knowledge of Aged Care Quality Standards and regulatory compliance
n
Leadership experience managing multidisciplinary teams
n
Experience managing operational budgets and workforce planning
n
Current National Police Check
n
Working With Children Check (if required)
n
Current Driver's Licence
n
