General Practitioner - Roaming Clinic + Telehealth (VR GP)
n
Proactive Care is seeking experienced VR GPs to deliver chronic disease management and multidisciplinary care for veterans and patients with complex, multi-system needs. The role blends a roaming clinic component, across clinic and community locations in your metro area, with remote telehealth case-conferencing.

This is not a standard role, and a long way from ten-minute, high-volume telehealth. You will provide clinical leadership within integrated Chronic Care Teams, building coordinated, evidence-based care plans.

REMUNERATION

You start on a guaranteed hourly rate while you understand the new patient base, then move to a share of billings once comfortable - a secure floor with real upside.

First 3 months (guaranteed): $****** per hour

After 3 months: 70% of billings - Independent contractor arrangement; bulk-billing across Medicare and DVA
n

THE ROLE
n
n
GP consultations for acute, chronic and complex conditions across clinic and community locations
n
Preventative health assessments and chronic disease care planning (GPMPs, TCAs)
n
Scheduled telehealth case conferences within multidisciplinary Chronic Care Teams
n
Prescribing, referrals and investigation management
n
n
Available across Adelaide, Sydney, Melbourne and Perth. Structured days each week, arranged around you.

REQUIREMENTS
n
n
MBBS or equivalent and current AHPRA registration
n
FRACGP or FACRRM (VR GP), or eligibility for a 19AB exemption in an eligible DPA location
n
Experience in chronic disease management and complex care coordination
n
Willingness to travel across metro clinic and community locations
n
n
Interest or experience in veteran health, DVA or the CVC Program is highly regarded but not essential; full onboarding provided.
