This is an excellent opportunity to relocate to the beautiful Wauchope region, located approximately 4 hours north of Sydney on the Mid North Coast of NSW, and join a dedicated Home Care team making a real difference in the community. We are seeking an experienced Clinical Care Manager - Home Care to provide clinical leadership, drive quality improvement initiatives, and support the transition to the new Support at Home program.

Position: Clinical Care Manager - Home Care

Employment Type: Full-time Permanent

Salary: $120,000 base salary + Super

Key Responsibilities
n
n
Provide clinical leadership and oversight across the Home Care program.
n
Drive clinical quality improvement initiatives and strengthen clinical governance.




n
Work closely with the Home Care Manager to implement and progress an approved action plan.
n
Lead Support at Home readiness activities and continuous improvement initiatives.
n
Mentor and support clinical staff to deliver safe, person-centred care.
n
Ensure compliance with aged care legislation, quality standards, and best practice.
n
n
About You
n
To be successful in this role, you will have:
n
n
Current AHPRA registration as a Registered Nurse.
n
A minimum of 3 years' Home Care management experience.
n
Strong understanding of the Australian Home Care and aged care sector.
n
Demonstrated experience leading clinical improvement and quality initiatives.
n
Excellent leadership, communication, and stakeholder engagement skills.
n
A cooperative approach with a passion for delivering exceptional client outcomes.
n
