The Complex Patient Care Navigator at The Sutherland Hospital, part of SESLHD, coordinates comprehensive assessment, care planning and discharge for older and vulnerable inpatients with complex needs.

Working with the Deputy Director of Nursing and Midwifery, you will optimise patient flow, coordinate resources, and advocate for patients and families, ensuring protected, effective care throughout admission to discharge.
