Recovery Alliance Care – Greensborough VIC

We are looking for an experienced Client Care Coordinator to join our team in a Casual role five days a week.
n
You will have the flexibility to work from home and from our Office in Greensborough.
n
You will be:
n
n
Responsible for planning, developing and coordinating services
n
Responsible for interpreting participants' NDIS plan
n
Assisting participants in the planning and implementation of services
n
Helping participants gain skills to allow them to become independent in managing their own supports
n
Monitoring and reviewing plans and goals in consultation with participants on an ongoing basis.
n
Arrange any necessary assessments that participants may require.
n
n
About you:
n
n
Have an enthusiastic and compassionate nature
n
Excellent communication and interpersonal skills
n
Have experience as a Client Care Coordinator or a similar role working within the NDIS setting
n




Minimum of less than 1 year experience is required.
n
Mental Health experience essential.
n
Ability to liaise and build rapport with participants, families and support workers.
n
Able to show knowledge of NDIS and its framework
n
Experience with both mental health and disability participants
n
Exceptional time management and organisational skills with the ability to priorities competing demands.
n
Hold NDIS Screening Check
n
NDIS Screening Check can be obtained after hiring
n
Currently hold or willing to obtain a National Police Check and Working with Children Check
n
A valid driving license and access to a vehicle is required
n
n
