Fairmont Aged Care is seeking a full time DT/Leisure-Lifestyle Officer to complement the delivery of our extensive activities program for our residents, ensuring excellence in Aged Care. Providing a flexible working arrangement, the ideal applicant will be dedicated to administering an interactive activity program, assisting the leisure-lifestyle team. Experience in aged care is preferred as is a commitment for continuous improvement and innovation.

Qualifications/Requirements:

Bachelor of Health Science (Leisure & Health) / Bachelor of Health Science (Therapeutic Recreation)
n
High Level Communication Skills
n
Proven leadership and teamwork skills
n
Effective interpersonal skills
n
Proficient, analytical documentation skills
n
n
Major Duties:
n
n
Facilitate the leisure lifestyle program, including group and individual activities for residents, including people living with dementia
n
Monitor and evaluate resident participation, progress, and interest level in recreation striving for continuous improvement
n
Complete daily documentation for the leisure lifestyle program, including group and individual activities
n




Assist in the review of lifestyle assessments as part of the overall care plan
n
Respect and assist residents to practice their cultural, religious, intellectual, physical and social beliefs. Respect resident's rights, dignity and privacy at all times.
n
Liaise with and maintain a valuable working relationship with the community, other professionals' and organisations in the provision of service
n
Assist in developing the monthly recreation program and producing monthly facility newsletter
n
Schedule, plan and organise various lifestyle events on the yearly calendar
n
Assist in facilitating resources required for activities that are added to the program
n
Assist in completing any surveys or auditing tools as required
n
Conduct 1:1 sessions with individual residents providing emotional and social support
n
Assist in escorting residents to medical and other appointments ensuring their needs are met
n
Assist with documentation and other requirements and duties ensuring the outcomes of the Aged Care Quality Standards are always met
