About The Role
n
This role plays a vital part in ensuring safe, timely, and effective discharge planning for patients. Working as part of a multidisciplinary healthcare team, the Discharge Planner supports patients and their families by coordinating care, services, and resources to promote continuity of care following discharge. This is a full time relieving role Monday to Friday.
n
Your main duties will include:
n

Assessing patient needs and discharge readiness
n
Coordinating community services and support agencies
n
Collaborating with clinical staff, patients, and families
n
Advocating for patient-centred, safe discharge outcomes
n
n
About You
n
n
Current AHPRA registration
n
Relevant clinical knowledge,



skills and experience developed in an acute care environment
n
Willingness to educate and provide advice to patients and nursing staff
n
Demonstrated critical thinking, problem solving and planning skills
n
Demonstrated advance communication and interpersonal skills relating to internal and external customers
n
Experience in discharge planning
n
