The Smith Family is seeking an On PAR Case Worker to provide holistic case management for students and families within the On PAR program in Mt Druitt, NSW. This is a temporary full time position through December **** with hybrid working options.
You will maintain a caseload, collaborate with schools and local service providers, manage data for evaluation, and support the design and delivery of client-centred services. A Social Work degree and NSW Driver's Licence are required.
