About the Organisation
n
Based in the outer eastern suburbs, you will be working with a provider of aged care assessment services, dedicated to helping older Australians remain independent and maximise their quality of life.
n You will be working collaboratively with clients, families, carers, and service providers to connect people with the right support services at the right time, by providing a comprehensive in-home assessment and reviews as needed.

The Role
n
You'll conduct autonomous, person-centred assessments to determine the care and support older Australians need to live safely and independently at home.
n
Key Responsibilities:
n
n
Conduct timely, holistic assessments that enhance independence and quality of life.
n
Liaise with families, carers, and service providers as key stakeholders in the assessment process.
n
Manage your assessment schedule to meet Department of Health timelines and quality standards.
n
Maintain accurate documentation and reporting.
n
n
Note: This role involves travel within the inner southern metro region.
n
What We're Looking For
n
n
Must have completed Comprehensive Assessor certification and have recent experience working as a Clinical / Comprehensive Assessor within the Single Assessment Service
n
Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy, Allied Health, or Social Work
n
Strong knowledge of aged care legislation, CHSP, and Support at Home
n
Excellent communication, documentation, and interpersonal skills.
n
Valid Australian driver's licence and a fully insured vehicle.
n
Availability for at least 3 days per week
n
