Central Coast Local Health District is seeking an experienced mental health clinician for a permanent full time role in the Care Coordination Team on the Central Coast. Lead clinical practice across community centres and support high-quality, coordinated care for adults with moderate to severe mental illness.
You will provide clinical leadership, coordinate referrals, monitor governance and work with an integrated team to improve service delivery and consumer outcomes.
