Clinical Care Coordinator – VIC | Community Nursing

As the clinical leader for our Victorian community nursing services, you will work closely with our Registered Nurses, Enrolled Nurses, operational teams and wider leadership group. This is an on-site role based from our Werribee Head Office, with travel across Victoria as required to support our community nursing services.

Key responsibilities
n
Leading and clinically supporting our Victorian community nursing team
n
Providing clinical supervision, guidance, coaching and support to RNs and ENs
n
Building the capability and confidence of nurses working in the community
n
Completing comprehensive nursing and clinical risk assessments
n
Developing and reviewing nursing care plans and clinical support plans
n
Supporting protected delegation and assessing competency for clinical tasks
n
Overseeing medication management, PRN protocols and high-intensity supports
n
Providing clinical guidance for participants with complex health needs
n
Responding to clinical deterioration, incidents and safeguarding concerns
n
Supporting the safe onboarding of new participants requiring nursing or complex care
n
About you
n




Current unrestricted AHPRA registration as a Registered Nurse
n
Strong recent experience in community nursing, disability, complex care and/or aged care
n
Experience supervising, coordinating or leading nurses
n
Excellent clinical assessment and care-planning skills
n
Strong medication-management knowledge
n
Experience supporting people with complex clinical needs
n
Sound understanding of NMBA standards, delegation, supervision and scope of practice
n
Confidence assessing clinical competency and supporting the development of other nurses
n
Strong clinical judgement and risk-management skills
n
Excellent communication and relationship-building skills
n
Monday to Friday, 9:00am – 5:00pm
