Mecwacare is seeking a Care Partner to manage a designated caseload of Support at Home participants in the Ballarat region. The role blends community visits with time in our Ballarat office, focusing on person‐centred care planning and service coordination.

You will onboard participants, develop and review care plans, and coordinate services to meet complex needs while maintaining solid relationships with clients and providers. Travel across the region is required.
