The Role
n
We are looking for an experienced and passionate Chronic Disease Management Nurse to join our Elizabeth Medical Centre.
n
Working closely with GPs, nurses, specialists and allied health professionals, you will provide patient-centred chronic disease management and preventative healthcare services. You'll prepare care plans, conduct health assessments, coordinate multidisciplinary support and empower patients to take greater ownership of their health.

Your Responsibilities
n
n
Prepare GP Management Plans and Team Care Arrangements in collaboration with patients and GPs.
n
Gather patient histories, biometrics and information relating to chronic conditions.
n
Help patients set achievable health goals and develop practical action plans.
n
Coach and motivate patients to make sustainable health and lifestyle changes.
n
Conduct preventative health assessments, including heart health, diabetes risk, ***** and 75+ assessments.
n
Complete health assessments for Aboriginal and Torres Strait Islander peoples, veterans, refugees and people with intellectual disability.
n
Provide ongoing monitoring and support to eligible patients.
n
Perform clinical services such as ECGs, spirometry, ABI testing and urinalysis.
n
Coordinate care between GPs, specialists and allied health professionals.
n
Identify eligible patients and support proactive patient outreach.
n
Build relationships with GPs and nursing teams to increase appropriate patient referrals.
n
Maintain accurate clinical records and ensure compliance with Medicare requirements.
n
Support other nurses and non-clinical team members in understanding chronic disease management services.
n
Contribute to clinical quality, patient safety and continuous improvement.
n

About You
n
You are a proactive and compassionate nurse who is passionate about preventative healthcare and supporting patients to better manage their health.
n
You Will Bring
n
n
Current AHPRA registration as a Registered or Enrolled Nurse.
n
At least two years' experience providing direct clinical nursing care is preferred.
n
Previous experience within primary healthcare is highly regarded.
n
Experience in chronic disease management, health assessments or care planning is preferred.
n
Knowledge of health promotion and disease-prevention principles.
n
An understanding of Medicare requirements and billing processes.
n
Strong written and verbal communication skills.
n
Excellent organisation and time-management abilities.
n
The confidence to educate, motivate and positively influence patients.
n
A collaborative approach to multidisciplinary patient care.
n
A genuine interest in improving healthcare quality and patient outcomes.
