We're seeking a Registered or Enrolled Nurse to join our MaroubraMedical Centre, where you'll support GPs and patients through chronic condition management and preventative healthcare. You'll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full-time opportunities available.

What You'll Do:

Prepare Care Plans and Team Care Arrangements
n
Support patients in managing chronic conditions and improving health behaviours
n
Conduct health assessments and promote preventative care
n
Collaborate with GPs and multidisciplinary teams
n
Manage administrative tasks related to CCM appointments
n
n
About You:
n
n




AHPRA registered Enrolled or Registered Nurse
n
Prior experience in Chronic Condition Management preferred
n
Strong communication and organisational skills
n
Friendly, proactive, and patient-focused approach
n
