About the Role
n
Community Care Hub Australia Pty Ltd is currently seeking dedicated and compassionate Disability Support Workers for an immediate start across the Greater Western Sydney region.
n
As a Disability Support Worker, you will play an important role in supporting participants to achieve their individual goals, develop independence and actively engage with their local communities.
n
You will provide personalised, respectful and person-centred support to individuals with a range of disabilities and support needs, including:
n
n
ADHD
n
Autism Spectrum Disorder (ASD) – Levels 1, 2 and 3
n
Acquired Brain Injury (ABI)
n
Global Developmental Delay (GDD)
n
Multiple Sclerosis (MS)
n
Down Syndrome
n
Intellectual and psychosocial disabilities
n
Other complex and individual support needs
n
n
What You'll Be Doing
n
Your responsibilities may include:
n
n
Assisting participants with daily living activities, including personal care, meal preparation and household tasks
n
Supporting participants to attend appointments, social activities and community engagements
n
Encouraging independence, skill development and achievement of individual goals
n
Providing respectful and person-centred care in accordance with each participant's individual support plan
n
Advocating for participants' needs, choices, rights and dignity
n
Completing accurate progress notes, incident reports and other required documentation
n
Providing feedback and relevant updates to Service Coordinators and other stakeholders
n
Working collaboratively with families, allied health professionals and other support servicesAssisting with the implementation of individualised care and support plans
n
Responding appropriately to changing participant needs and behaviours
n
Following all relevant organisational policies, procedures, NDIS Practice Standards and workplace health and safety requirements
n

What We're Looking For
n
To be successful in this role, you will ideally have:
n
n
Certificate III in Individual Support, Disability Support or a related qualification (preferred)
n
Experience providing person-centred care and support to people with disabilities
n
Strong communication and interpersonal skills
n
The ability to build positive, professional and trusting relationships with participants
n
A genuine passion for supporting people to achieve greater independence and quality of life
n
Patience, empathy and a respectful approach to supporting others
n
Robust problem-solving skills and the ability to respond effectively to challenges
n
The ability to work independently as well as collaboratively within a team
n
n
Essential Requirements
n
n
Current National Police Check
n
Current First Aid and CPR Certificate
n
Current Working with Children Check (WWCC)
n
Current NDIS Worker Screening Check
n
Completed NDIS Worker Orientation Module: "Quality, Safety and You"
n
Current unrestricted Australian driver's licence
n
