As a Registered Nurse – Home Care, you will provide high-quality, person-centred clinical care to participants across South East Tasmania, supporting local communities while enjoying a rewarding and autonomous role.



This position requires flexibility to travel as needed to meet participants' needs.
n
Offering a high level of autonomy and variety, this role provides the opportunity to build strong, meaningful relationships with participants and their support networks while delivering care that makes a genuine difference.

This is a casual position with the potential to transition to part-time for the right candidate. The successful applicant must be available to provide support to participants across Bruny Island, Kettering, Snug, Oyster Cove and Margate.
n
We are seeking a Registered Nurse who is committed to delivering holistic, culturally responsive care and maintaining safe, evidence-based practice. You will be supported by a collaborative team environment and a strong clinical governance framework, enabling you to provide exceptional care and achieve positive outcomes for participants.
n
No two days are the same! As Registered Nurse – Home Care you will:
n
n
Conduct comprehensive clinical assessments and develop, implement, and review individualised care plans in collaboration with participants and the multidisciplinary team.
n
Deliver a range of complex nursing care in the home, including wound care, medication management, chronic disease management, and end‐of‐life support.
n
Monitor participant health, respond to changes, and elevate clinical concerns in line with governance frameworks.
n
Collaborate with Care Partners, allied health, GPs, and external providers to ensure coordinated, person‐centred care.
n
Provide education and support to participants,



families, and support workers, promoting safety, independence, and wellbeing.
n
Maintain accurate clinical documentation and ensure compliance with professional standards, legislation, and quality and safety requirements.

About You
n
You'll thrive with us if you have:

Essential
n
n
Current registration as a Registered Nurse with AHPRA with no restrictions.
n
Minimum two years post‐registration clinical experience, ideally in aged care, community nursing, or home care.
n
Competence in clinical assessment, care planning, and delivery of complex nursing care in community settings.
n
Knowledge of the Aged Care Quality Standards, relevant legislation, clinical governance, risk management, and incident reporting.
n
Current driver's licence, First Aid/CPR certification, and ability to travel to participants' homes.
n
Strong communication skills and ability to maintain accurate, compliant clinical documentation.
n

Desirable
n
n
Experience with home care frameworks (Home Care Packages, CHSP, Support at Home) and supporting participants with complex clinical needs, including dementia.
n
Familiarity with electronic clinical systems and participation in quality improvement or clinical governance activities.
n
Postgraduate or specialist training in aged care, wound care, chronic disease management, or palliative care.
