Clinical Care Coordinator The Opportunity

We're looking for a clinically strong and motivated Registered Nurse to step into a newly created Clinical Care Coordinator position with an established not-for-profit aged care provider.
n
This is a great opportunity for someone who enjoys being hands-on clinically while also supporting and developing others. You'll play an important role in maintaining high standards of care, mentoring the clinical team and helping drive positive outcomes for residents.
n
Ideally, you'll bring around three years of residential aged care experience. However, we're equally interested in speaking with motivated RNs who may have less experience but demonstrate strong clinical skills, initiative and a genuine drive to progress their career.

What You'll Be Doing
n
n
Provide day-to-day clinical guidance to RNs and care staff
n
Support clinical assessments, care planning and reviews
n
Monitor resident outcomes and identify areas for improvement
n
Mentor staff and promote high standards of clinical practice
n
Support incident management and clinical follow-up
n
Assist with maintaining accurate and compliant documentation
n
Contribute to quality and continuous improvement initiatives
n
Promote safe, person-centred care across the team
n

About You
n
n
Current unrestricted AHPRA registration as a Registered Nurse
n
Residential aged care experience, ideally around 3 years
n
Strong clinical assessment and decision-making skills
n
Leadership experience advantageous but not essential
n
Confident, proactive and willing to take initiative
n
Strong communication and relationship-building skills
n
Genuine ambition to develop within clinical leadership
n
n
