About the Role
n
Chosen Family is seeking compassionate and participant-focused Enrolled Nurses (ENs) to provide high-quality nursing and disability supports across home, community, respite, and Supported Independent Living (SIL) environments.
n
The EN will work collaboratively with participants, families, Registered Nurses, allied health professionals, and support teams to deliver safe, person-centred, and trauma-informed care that promotes participant wellbeing, independence, dignity, and quality of life.
n
Key Responsibilities
n
Clinical Nursing Support
n
n
Deliver safe and effective nursing supports within EN scope of practice
n
Assist participants with:
n
Medication administration and monitoring
n
Personal care and hygiene supports
n
Mobility and manual handling supports
n
Continence management
n
Diabetes monitoring
n
Vital observations
n
Nutrition and hydration support
n
Support delegated clinical procedures including:
n
Wound care
n
Catheter care
n
PEG feeding assistance
n
Bowel care
n
Pressure area care
n
Stoma care support
n
Monitor participant health and wellbeing
n
Identify signs of deterioration, behavioural escalation, or health risks
n
Escalate clinical concerns appropriately to Registered Nurses or emergency services
n
Support infection prevention and control procedures
n
n
Participant-Centred Support
n
n
Promote participant dignity, choice, and autonomy
n
Support participants to work towards individual goals and care outcomes
n
Encourage independence and capacity-building
n
Support community access, appointments, and social participation
n
Build positive and therapeutic relationships with participants
n
Maintain respectful, culturally safe, and trauma-informed support practices
n
Support participant emotional wellbeing and confidence-building
n
n
Care Planning & Service Coordination
n
n
Follow participant care plans, behaviour support plans, and risk assessments
n
Contribute to participant reviews and care discussions
n
Work collaboratively with Registered Nurses and multidisciplinary teams
n
Support continuity of care across shifts and service settings
n
Escalate incidents, safeguarding concerns, and hazards appropriately
n
Assist with implementation of positive behaviour support strategies where required
n
n
Stakeholder Engagement
n
n
Liaise professionally with participants, families, guardians, and carers
n
Communicate effectively with Support Coordinators and allied health professionals
n
Participate in handovers, case discussions, and team meetings
n
Maintain skilled, compassionate, and responsive communication
n
Support positive participant and family experiences
n
n
Compliance & Documentation
n
n
Maintain accurate progress notes and clinical documentation
n
Complete medication records, incident reports, and shift notes
n
Ensure supports align with NDIS Practice Standards and organisational policies
n
Maintain participant confidentiality and privacy
n
Participate in audits, compliance activities, and ongoing training
n
Ensure safe work practices and adherence to infection control standards
n
n
Essential Criteria
n
Diploma of Nursing
n
n
Current AHPRA Registration as an Enrolled Nurse
n
Current NDIS Worker Screening Check
n
Current Working With Children Check
n
Current First Aid & CPR Certificate
n
Valid Driver's Licence and reliable vehicle
n
Strong communication and interpersonal skills
n
n
Desirable Criteria
n
n
Experience in disability or community nursing
n
Experience supporting participants with high-intensity supports
n
SIL or complex care experience
n
Mental health or psychosocial disability experience
n
Understanding of trauma-informed and person-centred practice
n
