We're excited to welcome a Clinical Nurse to our Community Connect team!
The Clinical Nurse role within Community Connect leads and delivers person centred nursing care at an advanced level based on the Nursing and Midwifery Board of Australia Registered Nurse Standards for Practice to help facilitate a safe transition from hospital to the community, particularly for people who are deemed ‘at-risk'. The Clinical Nurse is expected to practice autonomously and provide leadership in clinical decision making. They have the responsibility for professional leadership in the absence of the Nurse Grade 7 and an opportunity to work collaboratively to lead practice changes and clinical innovation.
Key Responsibilities
- Provide nursing aservices in health service settings.
- Responsible for a portfolio with a focus on clinical care/leadership, management, education and/or research.
- Manage, prioritise and allocate incoming referrals to ensure timely, responsive action.
- Assess individual patient's previous and current levels of function and determine appropriateness of their existing community-based support services with recommendations as required.
Remuneration value up to $10 727 p.m., comprising salary between $4390.00 - $4700.90 p.f., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (NRG6(1)) (2x Fixed term temporary positions available from 14/09/2026 until 20/12/2026).
