About the role

HomeCaring Pty Ltd is seeking an experienced and compassionate Aged Care Support Team Leader to join our dedicated team in Parramatta, NSW. This is a full-time position that offers the opportunity to lead and support our aged care team whilst making a meaningful difference in the lives of older adults and individuals with disabilities. As a Team Leader, you will play a vital role in ensuring the delivery of high-quality, person-centred care and support services. This position combines hands-on care delivery with team management responsibilities, allowing you to directly impact both clients and staff members. The role is based in Parramatta NSW 2150.

Key responsibilities

- Lead and supervise a team of aged care support workers, providing guidance, mentoring and performance management to ensure quality service delivery

- Coordinate and deliver direct care and support services to clients, including personal care, mobility assistance and social engagement activities

- Ensure all care is provided in accordance with individual care plans, client preferences and dignity and respect standards

- Maintain comprehensive and accurate client records and documentation, ensuring compliance with aged care regulations and organisational policies

- Conduct regular team briefings and training sessions to promote best practice, safety protocols and professional development

- Manage rostering, leave management and scheduling to ensure adequate staffing levels and continuity of care

- Communicate regularly with clients, families and healthcare professionals to ensure coordinated and responsive care

- Monitor health and wellbeing outcomes for clients and escalate concerns to appropriate healthcare providers or management

- Promote a positive, inclusive workplace culture that values teamwork, respect and continuous improvement





- Ensure compliance with aged care legislation, privacy laws, occupational health and safety standards and organisational policies

What we're looking for

- Relevant qualification in aged care, disability support or a related health and community services field (Certificate III or higher in Aged Care, Disability Support or equivalent)

- Demonstrated experience working in aged care or disability support services, with at least 2-3 years in a supervisory or leadership role

- Strong knowledge of aged care standards, person-centred care principles and legislative requirements including the Aged Care Quality Standards

- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, team members and external healthcare professionals

- Proven ability to manage and motivate a team, fostering a supportive and collaborative workplace setting

- Sound organisational and time management skills, with the ability to prioritise and manage multiple responsibilities

- Compassion, patience and empathy, with a genuine commitment to improving the quality of life for older adults and people with disabilities

- Current First Aid and CPR certification, or willingness to obtain these

- Working With Children Check clearance and National Police Check (or ability to obtain these)

- Reliable, professional attitude with a strong commitment to duty of care and ethical practice





- Experience with aged care management systems and digital documentation tools is desirable
