Your new role:

As an Aboriginal and Torres Strait Islander Health Worker – Chronic Disease, you will play a key role in improving health outcomes for Aboriginal and Torres Strait Islander people and families across the Logan community. Working within a multidisciplinary team, you will provide culturally appropriate chronic disease support, health education and care coordination while building strong connections between clients, families, community and health services.

Key Responsibilities

- Provide culturally appropriate chronic disease support, education and health promotion.
- Connect Aboriginal and Torres Strait Islander people and families with appropriate health and social services.
- Build strong relationships with community, healthcare providers and key stakeholders to improve health outcomes.
- Support clients to take ownership of their health management and access appropriate chronic disease services.

About you:

You're a culturally knowledgeable and community-focused health professional who can build trusted relationships and work confidently with Aboriginal and Torres Strait Islander people, families and multidisciplinary teams. You'll bring strong communication, coordination and chronic disease knowledge to this important role.

Skills & Experience

- Strong understanding of Aboriginal and Torres Strait Islander culture and community needs.
- Clinical knowledge and experience in chronic disease, including diabetes, respiratory and cardiac care.
- Strong interpersonal, communication, planning and problem-solving skills.
- Ability to work autonomously while contributing effectively within a multidisciplinary team.
