Our Hospital in the Home (HITH) service enables patients who would otherwise require hospital admission to safely receive acute-equivalent treatment in their own homes, workplaces, residential aged care facilities or through telehealth and virtual care.

This is more than a model of care. It's a significant shift in how healthcare is delivered, bringing hospital-level care closer to where patients feel most comfortable.

About the Opportunity

We're seeking experienced Clinical Nurses to join our growing Hospital in the Home (HITH) team based at St Vincent's Private Hospital Sydney.

As a HITH Clinical Nurse, you'll be part of a progressive multidisciplinary team delivering high-quality, evidence-based care outside the traditional hospital setting. Working closely with Clinical Leads, Visiting Medical Officers, allied health professionals and community providers, you'll help patients receive safe, coordinated and compassionate care in their own environment.

Key Responsibilities

- Conduct comprehensive patient assessments and clinical reviews.
- Develop and implement tailored care plans to support optimal patient outcomes.




- Deliver advanced nursing interventions, including IV therapies and complex wound care.
- Monitor patients with acute and complex healthcare needs and escalate care when required.
- Coordinate referrals, admissions, care transitions and discharge planning.
- Collaborate with a multidisciplinary team to ensure high-quality, integrated care.
- Support patients, families and carers through education and care planning.
- Participate in quality improvement and service development initiatives.
- Provide clinical guidance and mentorship to new and junior team members.

About You

You're an experienced Registered Nurse who thrives in an autonomous environment, enjoys building strong patient relationships and is passionate about delivering high-quality care beyond the hospital walls.

You Will Bring

- Current registration as a Registered Nurse with AHPRA.
- Minimum three (3) years' post-graduate nursing experience.
- Experience in community nursing and/or acute medical, surgical, emergency, ICU or critical care environments.
- Demonstrated experience caring for patients with complex clinical needs and multiple co-morbidities.
- Strong clinical assessment, care planning and problem-solving skills.
- Excellent communication and stakeholder engagement capabilities.
- Ability to work independently while contributing to a multidisciplinary team.
- Strong digital literacy and confidence using virtual care technologies.
- Current unrestricted NSW Driver's Licence.
- Full and unrestricted working rights in Australia.

Highly Regarded

- Hospital in the Home (HITH), community nursing, virtual care or care-at-home experience.
- Postgraduate qualifications in nursing or related specialty areas.
- Experience contributing to service innovation, quality improvement or clinical projects.
