Role details The Coronary Care Unit at the Mackay Base Hospital is seeking applications for a Registered Nurse (Heart Health) to join their team on a permanent basis. Your opportunity A Registered Nurse (RN) provides nursing services within the health service settings; and demonstrates competence in the provision of nursing care as specified by registration requirements, NMBA standards and codes, educational preparation, relevant legislation, and context of care. Responsible and accountable for delivering quality patient focused, evidenced based nursing care providing leadership, direction and support to other nursing staff and students and working collaboratively with the Interdisciplinary team across the Cardiac Units within the MHHS and external facilities via Telehealth. The opportunity to develop and gain valuable complex clinical experience in the areas of Heart Health patient/staff education, cardiac rehabilitation, heart failure and chronic disease management, working within a supportive multi-disciplinary team. About the Team The Heart Health Team (Cardiac Rehabilitation) sits within the Cardiac Services Department at the Mackay Base Hospital and work closely with the Heart Failure Team, Coronary Care unit, Cardiac Catheter Laboratory, Cardiac Rapid Access Service, Clinical Measurements Unit and Allied Health Team. We're committed to the optimisation of health outcomes for people with a range of cardiac conditions ensuring quality of life for the client, their families, and carers by the delivery of quality, coordinated and integrated multidisciplinary care across services,



settings, and time. This service provides Education and support to patients and their families, cardiac rehabilitation and secondary prevention. This is further defined as Phase 1, Phase 2 and Phase 3 as aligned with the National Heart Foundation Guidelines. Whilst the Heart Health Team and the Heart Failure Team work closely together, the heart failure Education and management programs are provided by the Heart Failure nursing team, across the continuum from hospital to home. Nursing and Allied health staff members perform consults with clients in the acute care settings of Mackay Base Hospital as well as provide ongoing follow up care in the community by telephone, Pre and post assessments to assess for suitability to be included in the Cardiac Rehabilitation Exercise Program, and outpatient/Telehealth clinics.
