Role Purpose

The Support Worker provides person-centred support to participants of the National Disability Insurance Scheme (NDIS) - Caulfield North - to promote independence, community involvement, and wellbeing. This role delivers high-quality, respectful, and responsive support that aligns with each participant’s goals and needs, whilst upholding the values and standards of the organisation.

Key Responsibilities

- Participant Support and Engagement
- Provide support to participants in their home and community environments, based on their individual support plans.
- Assist with daily living activities such as personal care, meal preparation, mobility, and medication reminders (where appropriate).
- Support community participation and social inclusion, including attending outings, appointments, and activities.
- Promote participant choice, control, and dignity in all aspects of support.
- Foster the development of skills and independence where possible.

Documentation and Communication

- Maintain accurate and up-to-date shift notes, incident reports, and participant records.
- Report any concerns regarding participant wellbeing or safety to the appropriate supervisor.
- Communicate respectfully and effectively with participants, families, and other stakeholders.
- Support the implementation of behaviour support plans and care protocols as required.
- Compliance and Best Practice
- Comply with all relevant organisational policies, procedures, and the NDIS Practice Standards.
- Ensure support is delivered in line with the participant’s plan and within the scope of your role.
- Uphold privacy and confidentiality in line with legal and organisational standards.
- Follow manual handling, infection control, and food safety protocols.




- Support the participant's rights and report any safeguarding concerns in line with mandatory reporting obligations.

Health and Safety in the Workplace

- Adhere to all workplace health and safety (WHS) policies and procedures.
- Ensure compliance with WHS legislation and participate in risk assessments where required.
- Use personal protective equipment (PPE) appropriately.
- Immediately report hazards, incidents, injuries, or concerns to your supervisor.
- NDIS Worker Screening and Professional Conduct
- Maintain a current NDIS Worker Screening Check and Working with Children Check.
- Participate in ongoing mandatory training, including the NDIS Worker Orientation Module and infection control training.
- Represent the organisation professionally and ethically at all times.

Key Selection Criteria

Essential:

- 1 year Experience supporting clients with behaviour of concern is a MUST
- Genuine passion for supporting people with disability to achieve their goals.
- Robust interpersonal and communication skills.
- Ability to work autonomously and within a team.
- Understanding of person-centred support and duty of care.
- Flexibility to work varied shifts, including weekends and public holidays.
- Current NDIS Worker Screening Check and Working with Children Check (or ability to obtain).
- Valid First Aid and CPR Certificate.
- Current Driver’s Licence and reliable vehicle (if required for the role).

Desirable:

- Certificate III in Individual Support (Disability) or equivalent.
- Experience supporting individuals with physical, intellectual, or psychosocial disability.
- Understanding of the NDIS, its goals, and the rights of participants.
- Mental health first aid or positive behaviour support training.

Pay: From $36.00 per hour
