Your new role:
As a Clinical Nurse with Rehab@Home Bayside, you will provide advanced clinical care, leadership and coordination for clients with complex needs receiving restorative and rehabilitation care in their homes and residential settings. Working autonomously within a multidisciplinary team, you will support high-quality outcomes while contributing to education, quality improvement, research and service development.

Key Responsibilities

Provide advanced clinical assessment, care planning and coordination for clients with complex needs.
Coordinate restorative and rehabilitation-focused care across community and residential settings.
Contribute to education, quality improvement, research and clinical governance initiatives.

About you:
You are an experienced Registered Nurse who brings strong clinical judgement, leadership and problem-solving skills to community-based care. You are passionate about restorative practice and supporting older people with complex care needs to achieve their best possible outcomes.
