Job title: Occupational Therapist, Aged Care Contractor (Brisbane)

Job description:

Rising Phoenix Care is a Brisbane based aged care provider currently completing registration with the Aged Care Quality and Safety Commission to deliver Support at Home services. As we prepare to launch, we are seeking an experienced occupational therapist to join our contractor panel and support clients across greater Brisbane in their own homes.

About the role

Conduct home based occupational therapy assessments for older clients under Support at Home, focusing on functional independence, and safe home environments

Deliver restorative and reablement focused interventions aligned with the Restorative Care Pathway

Recommend and coordinate home modifications, assistive technology, and mobility aids

Develop and review individualised care and therapy plans in collaboration with clients, families, and our care team

Provide clear,



timely clinical documentation to support funding, reporting, and continuity of care

About you

- Current AHPRA registration as an Occupational Therapist
- Experience working with older adults, ideally in aged care, community, or home based settings
- Confident conducting home safety and functional assessments independently
- Right to work in Australia
- Reliable transport and a valid driver's licence
- A current National Police Certificate, or willingness to obtain one
- Robust communication skills and a warm, person centred approach to care
