Care Partner will coordinate and oversee client care by assessing needs, developing personalised care plans, coordinating services, monitoring outcomes, and working closely with clients, families, and healthcare professionals to ensure quality, person-centred support.
Key Responsibilities
- Conduct comprehensive client assessments to determine needs, goals, and risks.
- Develop, implement, and review individualised care or support plans.
- Coordinate and monitor the delivery of services and supports in line with client goals and funding arrangements.
- Work closely with clients, families, carers, and health professionals to ensure quality outcomes.
- Maintain accurate case notes, documentation, and compliance with regulatory and organisational requirements.
- Support clients in navigating My Aged Care, NDIS, or other community support systems.
- Identify and respond to changes in client needs or risks promptly.
- Promote independence,
dignity, and wellbeing through person-centred and strengths-based practice.
- Participate in team meetings, supervision, and ongoing career development.
Skills and Qualifications
- Degree or Diploma in Nursing, Social Work, Community Services, Allied Health, or a related field.
- Previous experience in case management, care coordination, or community services.
- Sound knowledge of Aged Care, NDIS, or Support at Home programs.
- Excellent communication, problem-solving, and organisational skills.
- Ability to build trust and rapport with diverse clients and stakeholders.
- Current National Police Check, and NDIS Worker Screening Check (as required).
- Preferably with valid driver's license and access to a reliable vehicle.
