About the Role:

MediRecruit is currently recruiting for an exciting 50% work from home opportunity supporting aged care assessment services in East Melbourne, VIC.

In this role, you will conduct comprehensive aged care assessments to determine the care and support needs of older Australians. Working as part of a multidisciplinary team, you will undertake holistic assessments that consider clients' health, functional capacity, social circumstances, and overall wellbeing, helping them access appropriate aged care services and supports.

- Conduct comprehensive aged care assessments in accordance with relevant assessment frameworks and guidelines
- Assess clients' clinical, functional, cognitive, psychosocial, and environmental needs
- Develop recommendations and support plans to facilitate access to appropriate aged care services
- Complete detailed assessment documentation and reports
- Work collaboratively with multidisciplinary clinicians and stakeholders
- 50% remote/work from home role
- Full-time or part-time opportunities available (minimum 4 days per week)

About You:

- Current AHPRA registration
- Strong communication and clinical assessment skills
- Experience in aged care, community health or rehabilitation highly regarded
- Ability to work autonomously in a fully remote environment
- Excellent documentation and stakeholder communication skills
