Position Overview

Rembrandt Living is experiencing an exciting period of growth, and to complement this, we are seeking a compassionate and skilled Home Care Admissions Coordinator to join our home care team.

The Admissions Coordinator plays a critical role in welcoming and supporting current clients into our Support at Home (SAH), Commonwealth Home Support Program (CHSP) services, and Fees for Service (FFS).

This position manages the client intake process, including conducting home visits, preparing initial care plans, developing budgets, and ensuring all required agreements are completed. The role requires a strong client focus, organisational skills, and the ability to meet weekly onboarding targets.

Key Role responsibilities

- Promote Rembrandt Living Home Care as a preferred provider of service delivery choice for consumers and participants.
- Support quality assessment, care planning and service delivery processes,



consistent with evidence-based practice and positive case management principles within the Governance Framework.
- Subject matter expert in managing and coordinating Support at Home/Home Care Packages, Commonwealth Home Support Program (CHSP) and Fee for Service (FFS).
- Contributes to the development, design, delivery and review of an integrated suite of high-quality services that align our service delivery and systems of support with sector-wide home care.
- Builds trusted relationships with clients and their families, ensuring a positive first experience.
- Proactive, solutions-focused, and adaptable to client needs

About You

- Full time availability Monday - Friday
- Degree qualified in Human Services or related discipline or Certificate IV in Community Services
- A minimum of 1 year experience in coordinating community care or aged care services.
- Great attention to detail, communication skills, both written and verbal
- Strong organisational and time management skills
- Strong attention to detail with analytical and problem-solving skills
- A Current police clearance or willingness to obtain one
- A Current driver’s licence and access to a reliable vehicle
- First Aid and CPR Certificate (Mandatory)
