About the Role

The Practice Nurse plays and integral role in delivering high-quality, patient-centred clinical care across Ballarat Group Practice. Working collaboratively with General Practitioners and the broader multidisciplinary team, the Practice Nurse supports preventative health initiatives,



chronic conditions management, treatment services and care coordination activities that contribute to improved patient outcomes.

The role combines clinical expertise, patient advocacy and health education to ensure safe, evidence-based care is delivered in accordance with professional standards, accreditation requirements and organisational policies. The role contributes to continuous quality improvement and supports the delivery of efficient and accessible healthcare services across the practice.

Key Outcome Areas

Delivery of high-quality clinical care

Provide safe evidence-based nursing care that supports effective and positive patient outcomes across preventative health, clinical presentations and chronic conditions management. Apply clinical assessment skills, professional judgement and best-practice nursing principles to ensure patients receive timely, appropriate and high-quality care.

Proactive patient management and care coordination

Support proactive care through health assessments, care planning, follow-up activities and preventative health initiatives. Work collaboratively with patients, general practitioners and external providers to facilitate continuity of care and improve long term health outcomes.

Patient Education and Health Promotion

Promote health literacy and patient self-management by providing e4ducation, support and resources tailored to individual needs. Encourage participation in preventative screening, immunisation programs and chronic conditions management activities to support overall wellbeing.

Clinical Governance and Quality Improvement





Maintain accurate clinical documentation and contribute to safe clinical systems, infection prevention and accreditation requirements. Participate in quality improvement activities that enhance patients safety, clinical effectiveness and service delivery outcomes.

Practice Operations and Team Collaboration

Contribute to the effective operation of the practice through professional communication, teamwork and administrative support activities. Build positive working relationships with colleagues and stakeholders while supporting a collaborative and patient focused workplace culture.

Skills, Knowledge and Qualifications

Essential

- Current registration as a Registered Nurse with AHPRA

- Demonstrated experience in primary care, community health or comparable clinical environment

- Sound clinical assessment and patient management skills across a broad range of presentations

- Understanding of chronic conditions management, preventative health and care planning processes

- Strong interpersonal and communication skills with the ability to engage effectively with patients, colleagues and external stakeholder in a professional and respectful manner.

- Ability to work independently or as part of a team, contributing to a positive, collaborative workplace culture

- High level organisational skills with the ability to manage competing priorities and maintain attention to detail

- Demonstrated commitment to professional standards, continuous improvement and patient centred care

- Proficiency in the use of clinical and administrative systems
