Assessment Officer – In Home Care
Secondment | Full-Time | 12 Months
About the Role
We are looking for a motivated and passionate Assessment Officer to join our supportive and friendly In-Home Care team on a 12-month full-time secondment.
As an Assessment Officer, you will play an important role in supporting our customers to access the care and services they need to live safely and independently in their own homes.
You will conduct initial assessments for new In-Home Care customers, complete ongoing annual reviews and ensure all assessments, care plans and relevant documentation are completed accurately and within required timeframes.
The role will involve conducting assessments both over the phone and in customers' homes, working closely with customers, families and our multidisciplinary teams.
Key Responsibilities
- Complete initial customer assessments and develop care plans for In Home Care customers, ensuring their eligibility and support needs are accurately assessed.
- Conduct customer assessments and annual reviews in a respectful, professional and timely manner.
- Complete in-home visits as required and in accordance with current protocols.
- Manage a customer portfolio in line with team targets, service requirements and budgets.
- Ensure accurate and timely completion of assessments, documentation and customer records.
- Manage customer sign-ups and onboarding from end to end.
- Work collaboratively within a multidisciplinary team to deliver quality customer outcomes.
- Act as an escalation point for Customer Service Support on assessment and customer-related matters.
- Identify and respond to customer needs while maintaining a robust focus on quality,
safety and compliance.
About You
We are looking for someone who is customer-focused, organised and confident working independently while also being a strong team player.
You will ideally have:
- A minimum Certificate III in Aged Care or equivalent qualification.
- Demonstrated experience in case management, assessment or care coordination.
- Experience using client management systems such as AlayaCare, Salesforce and MAC is highly regarded.
- Excellent attention to detail and the ability to manage competing priorities and deadlines.
- Strong communication and interpersonal skills.
- A genuine commitment to supporting older people and delivering high-quality customer outcomes.
- Based in Melbourne’s South-East Suburbs with a current driver’s licence and willingness to travel across the region
- National Police Check (or willingness to obtain)
- Proof of working rights in Australia
