Embrace Community Care is seeking a motivated and caring person to join our team as a Team Leader in Western Sydney.

This is a outstanding opportunity for someone who is passionate about supporting people with disability and wants to develop their leadership skills within a supportive community services environment.

About the Role

As a Team Leader, you will help coordinate the day-to-day operations of our services and support our team to provide high-quality, person-centred care.

Your responsibilities will include:
Supporting and guiding Support Workers
Assisting with day-to-day service coordination
Communicating with participants, families and staff
Helping ensure supports are delivered according to participant needs
Supporting staff with day-to-day questions and concerns
Maintaining accurate records and documentation
Promoting a positive, respectful and inclusive environment
Following Embrace Community Care policies, procedures and NDIS requirements




Escalating concerns and incidents to management when required

About You

We are looking for someone who is:
Caring, reliable and responsible
A confident communicator
Organised and able to manage multiple tasks
Comfortable working as part of a team
Willing to learn and develop leadership skills
Passionate about supporting people with disability
Respectful of participant choice, dignity and independence
Able to work independently when required
