Job Description

NSW Health is seeking a Social Worker for the Out-Of-Home Care Health Pathway Program at Cranebrook Community Health Centre. The role involves trauma-informed assessments, care planning, and multidisciplinary collaboration to support infants, children and young people (0–18) in care.

You will deliver health assessments, manage health plans and provide psychosocial interventions while working with carers, caseworkers and health specialists to improve health outcomes.
