Dementia Support Australia, through HammondCare, seeks a Dementia Consultant to deliver the Staying at Home program across Hobart or Chermside. The role involves client screenings, carer sessions, and engagement activities within a multidisciplinary team to improve outcomes for people living with dementia and their carers.

You will apply clinical knowledge with empathy, adapt to diverse client needs, and contribute to continuous program improvement while travelling interstate as required.
