St Vincent's Hospital Melbourne is seeking a Community Case Manager for the APATT team in Kew.
The clinician will conduct comprehensive psychiatric assessment, support home visits, intake and triage, crisis support, and case management within a recovery-focused model.
Required are postgraduate qualifications, AHPRA registration, and at least five years of post-qualification mental health experience including care coordination.
