Job Description

As a Care Manager Team Leader, you'll supervise and coach a team of Care Managers, lift practice standards across the client journey, and keep a small caseload of your own so you never lose sight of the work on the ground. You'll be the person the team turns to when a client matter gets complex. and the person who makes sure our care management practice stands up to any audit.

Key Responsibilities
n
n
Provide day-to-day supervision, coaching and structured mentoring to Care Managers across care planning, documentation, risk monitoring and funding oversight.
n
Act as first point of escalation for complex client matters, safeguarding concerns and service risks, ensuring timely resolution and documentation.
n
Audit care plans, client reviews,
budgets and risk assessments to maintain accuracy, compliance and audit readiness at all times.
n
Oversee client onboarding, scheduled and event-based reviews, and care transitions including hospital admissions and discharges.
n
Monitor team KPIs, utilisation and funding alignment, providing regular one-to-one supervision and performance development support.
n
Manage a reduced personal caseload, modelling best practice in assessment, goal-based care planning and documentation.

About you
n
n
Relevant experience leading or managing a team (essential)
n
Extensive working knowledge of Home Care Packages, the Aged Care Quality Standards and Support at Home
n
Care Management experience within community or aged care services
n
Tertiary qualification in a health-related field (social work, allied health or similar) desirable
n
Sound understanding of quality management systems, auditing, risk mitigation and WHS
n
Strong analysis and problem-solving skills, with an ethical, person-centred approach
n
High-level computer skills, accurate data entry, and proven organisation and time management
