About the Role
n
As a NursePractitioner, youwill play akey role indelivering
preventativecardiovascular
healthcareacross a varietyof community settings.
n
Working autonomously,while supported byour cardiologists andexperienced clinical andoperations teams, youwill conduct
comprehensivecardiovascular
assessments, identify patients atrisk and facilitatetimely referral pathwayswhere required.
n
You'll workwithin:

You'll have ongoingclinical support fromexperienced cardiologistsand a
collaborativemultidisciplinary
teamwho are alwaysavailable when needed.

Key Responsibilities
n
n
Perform advanced
clinicalassessments
and examinations
n
Interpret diagnostic findingsand develop evidence-based management planswithin yourscope of practice
n
Collaborate closely withGPs, cardiologists and alliedhealth professionals
n
Assist with thecoordination of patientcare and
specialistreferrals
n
Support the deliveryof cardiac diagnosticservices within themobile clinic environment
n
Maintainaccurate clinical documentationand patient records
n
Represent The
HeartScreenprofessionally
across allcommunity healthcare settings
n
n

n
We're looking for an experienced NursePractitioner who enjoys working independently while being part of a highly supportive clinical team.
n
You are passionate about
preventativehealthcare,
confident in autonomous clinical decision-making, and committed to improving accessto cardiovascular careacross Australia.
n
You are approachable, compassionate, adaptableand excited to be part of an organisation making a genuine differenceevery day.

Endorsed Nurse
Practitionerregistration
with AHPRA
n
Minimum 3 years' experience practisingas a NursePractitioner (or
significantadvanced
practiceexperience)
n
Experience in cardiovascular, chronic disease,
emergency, primary care oracute care settings
n
Excellentclinical assessment anddiagnostic skills
n
Robust understanding ofcardiovascular diseaseprevention and management
n
Ability to workindependently across diversehealthcare environments
n
Excellent communicationand patient educationskills
n
Friendly, approachable andpatient-focused
n
Current Australian Driver'sLicence
n
Professional Indemnity Insurance
n
Australian work rightsat the timeof application
n
Willingness to travel(paid travel provided) to regional andinterstate sites whenrequired
n
n
Highly Regarded
n
n
Experience in preventativehealth or cardiovascularcare
n
Experience working inmobile, outreach orregional healthcare
n
Experience within generalpractice or
communityhealthcare
n
Interest in innovativemodels of healthcaredelivery
