Job Description
Senior Disability Support Worker - Practice Lead
n
As a Senior Support Worker at Cram, you'll play a vital role in supporting our Disability Support Workers, at various sites. Reporting to the Team Leader, you'll work closely with team members, assisting in the delivery of exceptional, person-centred care to empower the people we support. This role focuses on championing our Human Rights Framework and Person-Centred Active Support model, ensuring that the rights, privacy, and dignity of each person we support remains central to our service delivery.

Key responsibilities include:
n
n
Collaborating with DSWs to provide high-quality, person-centred support that encourages participant decision-making and meaningful choice.
n
Mentoring and coaching DSWs to enhance their skills, reinforce active support principles, and build team commitment to person-centred approaches.
n
Promoting participant independence by supporting daily living activities, fostering skills for self-care, and providing graded assistance aligned with individual goals
n
Leading efforts to strengthen community engagement, encourage independence, and advance Cram's strategic vision to enhance the quality of life for participants.
n
Assisting with rostering, coordinating administrative processes, and managing site assets and participant-specific equipment.
n
Ensuring respectful, culturally appropriate, and effective communication with participants, families, and support networks.

ABOUT YOU
n
You have a proven track record of team leadership within a disability support setting, focusing on service quality, participant empowerment, and fostering a cooperative team culture.
You have experience working with people with high intensity daily support needs and excel at managing high-pressure situations with a sense of calm and confidence. You have strong communication skills and empathy, and are able to build trusted relationships with Participants, families, and stakeholders.

ESSENTIAL CRITERIA
n
n
Demonstrated ability to supervise, coach and support a team to achieve goals.
n
Ability to work a rotating roster including day, afternoon and weekend shifts within a 4-week roster.
n
Proven experience working with people who have high intensity daily support needs.
n
Demonstrated experience in completing individual support plans and assessments.
n
Demonstrated commitment to fostering participant independence and maintaining strong relationships with both participants and team members.
n
Strong computer literacy, with Microsoft based programs and attention to detail
n
Australian Drivers Licence.
n
Valid Working with Children's Check & NDIS Worker Screen Check.
n
Evidence of 2 doses of Covid-19 Vaccination.
n
n
DESIRABLE
n
n
A relevant qualification within Disability support (Cert IV or higher).
n
n
