Job Description
St Vincent's Hospital Sydney is seeking a Social Worker - Residential Aged Care Placement Officer for a permanent part time role (32 hours/ week over five days) within Social Work. The role coordinates transfers to Residential Aged Care Facilities, supports patients and families, and helps improve transitions from hospital to RACFs.
