Job Description
St Vincent's Public Hospital Sydney is offering a permanent part‐time Social Worker – Residential Aged Care Placement Officer (32 hours per week). You will coordinate the RAC placement service, support transfers from hospital to RACFs, and work with a multidisciplinary team to optimise patient outcomes.
