Job Description
The Opportunity
n
Royal Freemasons Footscray is seeking a dedicated and experienced Clinical Care Coordinator to join our Residential Aged Care team on a casual basis.
n
Our Footscray home has a strong Vietnamese and Chinese community, and we are looking for an individual who will embrace and contribute positively to our diverse and culturally inclusive environment.

Working closely with the Facility Manager, residents, families, and multidisciplinary teams, you will provide clinical leadership and oversight to ensure the delivery of high-quality, resident-centred care.
n

Key Responsibilities
n
n
Provide clinical leadership, guidance, and support to nursing and care teams.
n
Ensure accurate, timely, and compliant clinical documentation that meets regulatory requirements.
n
Champion and oversee the delivery of person-centred care.
n
Monitor and support wound management and complex clinical care needs.
n
Assist with staffing coordination, including managing sick calls and arranging replacement shifts where required.
n
Promote best-practice clinical care and a culture of continuous quality improvement.
n
Conduct daily reviews of progress notes and work logs, providing feedback and follow-up with staff as required.
n
Monitor residents with palliative care requirements and complex medical conditions.
n
Liaise effectively with GPs, hospitals, allied health professionals, specialists, residents, and families to coordinate care and ensure positive outcomes.
n
Support compliance with Aged Care Quality Standards, clinical governance requirements, and internal quality processes.
n

About You
n
n
Current registration as a Registered Nurse with AHPRA.
n
A minimum of 6 years' experience as a Registered Nurse, preferably including experience in a Team Leader, Senior RN, or Clinical Leadership role.
n
Willingness to work Night Duty
n
Demonstrated experience working in residential aged care.
n
Strong knowledge and experience in Clinical assessments
n
Care planning and documentation
n
Wound care management
n
Palliative care
n
Dementia and behavioural support
n
Previous experience as a Clinical Care Coordinator, Senior Registered Nurse, Team Leader, or similar position.
n
A sound understanding of the Aged Care Quality Standards and clinical governance requirements.
n
Excellent communication, leadership, and problem-solving skills.
n
The ability to build positive relationships with residents, families, staff, and external stakeholders.

Mandatory Requirements:
n
Current AHPRA registration.
n
Valid NDIS Worker Screening Check and/or National Police Check.
n
