The Role
n
As Community Care Manager, you will lead an established Support at Home service, supporting your team to deliver high-quality, person-centred care to community clients.
n
Key responsibilities include:
n
n
Leading and supporting Care Partners and care staff
n
Overseeing care plans, budgets and service reviews
n
Supporting complex clients and escalations
n
Monitoring compliance, quality, documentation and risk
n
Managing incidents, complaints and performance matters
n
Driving continuous improvement and staff development
n
Working closely with clients, families and broader leadership teams
n
n
About You You may already be a Care Manager, or an experienced Senior Care Partner / Team Leader ready to step into a broader management role.
n
You will ideally have:
n
n
Strong Home Care / Support at Home experience
n
Experience leading or mentoring community care teams
n
Strong knowledge of Support at Home and the Aged Care Quality Standards
n
Experience with care plans, budgets and client reviews
n
Strong understanding of quality, compliance and riskRelevant Community Services, Aged Care or Nursing qualification
n
