Your new role:
As a Clinical Nurse with Rehab@Home Bayside, you will provide advanced clinical care, leadership and coordination for clients with complex needs receiving restorative and rehabilitation care in their homes and residential settings. Working autonomously within a multidisciplinary team, you will support high-quality outcomes while contributing to education, quality improvement, research and service development.
Key Responsibilities
- Provide advanced clinical assessment, care planning and coordination for clients with complex needs.
- Coordinate restorative and rehabilitation-focused care across community and residential settings.
- Contribute to education, quality improvement, research and clinical governance initiatives.
About you:
You are an experienced Registered Nurse who brings strong clinical judgement, leadership and problem-solving skills to community-based care. You are passionate about restorative practice and supporting older people with complex care needs to achieve their best possible outcomes.

- Advanced clinical knowledge and experience caring for older people with complex needs.
