Monash Health is seeking a Senior Clinician to lead the Aged Care Assessment Service (ACAS) for the Vic 2 Region SAS. You will guide triage, assessments and reviews, supervising a growing multidisciplinary team to ensure consistent, high‐quality care for older people.
The role offers flexibility with work from home options and a focus on person‐centred care, dignity, independence and reablement, while building partnerships with health, community and aged care services.
