About the role

Empower Living Pty Ltd is seeking a Care Manager – Disability Support Services to join our team in Narooma, NSW. This is a part-time position that offers a rewarding opportunity to make a real difference in the lives of people living with disabilities.

As Care

Manager, you will play a pivotal role in coordinating and managing support services, ensuring that our clients receive personalised, high-quality care that enables them to live with dignity and independence. This position is integral to our mission of empowering individuals within our community.

What you'll be doing

- Assessing clients' support needs and developing individualised care plans that align with their goals and aspirations
- Coordinating and scheduling disability support services to ensure clients receive timely and appropriate care
- Liaising with clients, families, carers and support workers to maintain effective communication and continuity of care
- Monitoring the quality and effectiveness of support services provided and making adjustments as needed
- Maintaining accurate and up-to-date client records and documentation in compliance with regulatory requirements
- Supporting clients in accessing community resources, advocacy services and additional support programmes
- Conducting regular reviews of care plans and progress towards client goals
- Contributing to a person-centred approach to disability support that respects client autonomy and choices
- Collaborating with multidisciplinary teams and external service providers to enhance client outcomes

What we're looking for

- Relevant qualification in social work, community services, aged care, disability support or a related field
- Current or previous experience working in disability support, community services or a related care environment
- Demonstrated knowledge of disability support frameworks, including the National Disability Insurance Scheme (NDIS)
- Strong communication and interpersonal skills with the ability to work effectively with diverse groups of people




- Excellent organisational and time management abilities, with capacity to manage multiple client cases
- Proficiency in using computer systems and managing electronic client records
- A compassionate and person-centred approach to service delivery
- Current Working with Children Check (or equivalent screening clearance) or willingness to obtain one
- Reliability, trustworthiness and a commitment to maintaining client confidentiality and safeguarding
- Preferred experience with care coordination systems and experience in a disability support or community sector setting
