About the role

We are seeking an experienced Care Manager to join our Blackburn team on a full-time basis. This is an opportunity to build meaningful relationships with clients and their families while making a real difference every day. You will work alongside a dedicated team that is passionate about delivering exceptional care.

Key responsibilities

- Develop, implement and review comprehensive care plans in collaboration with clients, families and multidisciplinary teams.
- Coordinate and manage the delivery of support services, ensuring quality standards and compliance with regulatory requirements.
- Liaise with external service providers, healthcare professionals and community organisations to ensure integrated care delivery.
- Maintain accurate and up-to-date client records and documentation in accordance with privacy and confidentiality protocols.
- Manage budgets and resources effectively to ensure efficient and sustainable service delivery.




- Ensure compliance with aged and disability care legislation, policies and best-practice standards.

What we're looking for

- A qualification in Aged Care, Nursing, Social Work, Community Services or a related discipline is highly regarded.
- Previous experience in aged care and case management is essential.
- Strong knowledge of Home Care Packages, Support at Home, and aged care case management is required
- Robust administration and computer skills, including experience with Microsoft Office and client management systems.
- Well organised, proactive, and able to manage competing priorities in a fast-paced environment.
